Polytrauma Program
VA’s Polytrauma System of Care provides high-quality interdisciplinary care to patients. Teams of physicians from every relevant field plan and administer an individually tailored rehabilitation plan to help the patient recover as much as possible.
Patients who are served by the Polytrauma System of care generally:
- Have sustained multiple physical, cognitive, and/or emotional injuries secondary to trauma;
- Do not require one-to-one staffing for medical or behavioral reasons;
- Do not require a ventilator to breathe;
- Have potential to benefit from rehabilitation; or
- Need an initial, comprehensive rehabilitation evaluation and care plan
Syracuse VA Medical Center maintains a Polytrauma Network Site (PNS) to provide continued, specialized outpatient care. PNS interdisciplinary rehabilitation teams include:
- Physiatrist (rehabilitation doctor)
- Social Work Case Manager
- Nurse Case Manager
- Physical Therapist
- Occupational Therapist
- Rehabilitation Nursing
- Speech and Language Pathologist
- Rehabilitation Psychologist
- Other consultative services as needed
PNS teams work with you to:
- Manage and coordinate your rehabilitation plan of care
- Provide treatment
- Assist with integration back to your home and community
- Identify community resources for care
- Provide regular clinical case management and family support
